Players:

I as a member of the Stoughton Football program do acknowledge receipt of and agree to the following rules:

I agree to display conduct both on and off the field that reflects only the higheststandards of sportsmanship, remembering at all times, that my conduct is a direct reflectionupon my parents, friends, the community of Stoughton, and the Stoughton FootballProgram which I represent.

I agree that school is important. My school attendance and performance can beused to determine my participation in games and practices.

I agree to refrain from the use of alcohol, tobacco, and other illegal drugsand not to associate with individuals who use any of the above named substances.

I agree to use only language which is socially acceptable.

I agree to listen to the direction of my coach and to show respect towardcoaches and referees at all times.

I understand that violations of any of these rules can lead to reduced playingtime or dismissal from the program.

Parents:

I as a parent/guardian of a Stoughton Football program player do acknowledge receipt of and agree to the following rules:

I agree to display conduct during the games that reflects only the highest standardsof SPORTSMANSHIP; using socially acceptable language, showing respect towardreferees, and respect to coaches, players, and other parents, regardless of their teamaffiliation.

I agree to show respect toward player's, coaches, and other parents, duringpractices and other team functions.

I understand that violations of the above rules could result in my being asked toleave the event and being barred from future attendance.

I agree to try to resolve any issue in an orderly manner. Concerns involving mychild, and their team or coach, should first be addressed by trying to talk about thoseconcerns with the coach(es). If this does not solve the situation, I will contact the BoardMember that is my Grade Representative to help rectify the problem.

I understand that the equipment which is issued to my child is on a rental basis andthat I am required to return all pieces, on the day specified at the end of the season. Theequipment will be in the same condition as issued, accounting for normal wear. If I neglectto return any equipment issued to my child, I understand that I will be billed for thereplacement cost of such equipment and agree to the payment of such.
